Update to HSE COVID 19 Vaccination Consent Form

There was feedback to the Vaccination Steering Group on  the HSE COVID 19 Vaccination Consent, paper format, in particular regarding Part 2 of the form which asked the person to tick ‘yes’ or ‘no’ in relation to receiving the vaccine.

This is now updated by the Vaccination Group today January 22nd. The new format is attached below and in the YourOTC.net  Vaccination Updates folder.

Two Records of Choice:

HSE Covid 19 Vaccination Form ( saved in Covid19 Vaccination Passport Pack)

Vaccination Consent records - updated January 22nd ( saved under Communications Supports for people offered Vaccine)

The completed form should be retained in each person’s file alongside the appropriate SMH consent record (see a choice of two records.

This depends on the person’s communication profile – which were issued earlier in the week as part of the SMH COVID 19 Vaccination Support Pack (i.e. the easy read resource pack).
